All prospective clients are invited to schedule a 15-20 minute free phone consultation to share your physical therapy and/or fitness goals and to determine if I am truly the right provider for you.
Physical Therapy - Initial evaluation (90 minutes) – $300*
Includes interview, movement and postural analysis, physical/clinical assessment, and instruction of initial home program, which may include exercise, activity modification, self-mobilizations, or recommended changes to your daily routine.
A superbill that can be submitted to your private insurance can be provided by request.
Physical Therapy - Follow-up visit (60 minutes) - $200*
Includes an individualized combination of one or more of the following: manual therapy, therapeutic exercise, neuromuscular re-education, ergonomic and body mechanics consultation, bracing/taping/recommendations on assistive devices or adaptive equipment, pain management counseling, health promotion coaching.
A superbill that can be submitted to your private insurance can be provided by request.
Physical Therapy for Pets
Pet evaluation (60 minutes) - $175*
Pet follow-up visit (40 minutes) - $100*
25% discount for pet/owner back-to-back sessions!
Health Coaching & Mentorship - Initial Consultation (60 minutes) - $125
Done virtually on a video call, this initial consultation includes a comprehensive interview that will help us determine specific goals related to healthy longevity, physical performance and fitness, or injury prevention followed by specific recommendations and action items to help achieve your goals.
Health Coaching & Mentorship - Sessions (50 minutes) - $75 virtual, $100 in-person
Sessions are 50-minutes in duration and can be done virtually or in-person, depending on your coaching and mentorship needs.
10% package discount for 5- and 7-session packages. 15% package discount for 9+-session packages.
Therapeutic LASER
Add on to follow-up (10-15 min added to any session) - $30
LASER only session (~30 min) - $75
*Additional travel fee will be added for locations >25 miles from the Portland Metro area.
Cancellation/re-scheduling policy:
- Please re-schedule your visits at least one day prior to your session.
- Re-scheduling while I am already en route to you will incur a $60 re-scheduling fee.
- Cancellation upon arrival to your session will incur the cost of the session.
Trifecta Physical Therapy accepts cash, check, credit card, Venmo, and Zelle.
- Payment is required at time of service for the initial evaluation.
- Monthly invoices can be provided for follow-up visits and add-on services.
- A superbill that can be used to submit a claim to your insurance will be provided by request for physical therapy services only.
